Requirements:

a) Mail completed entries (Incomplete forms will not be accepted), copies of the dog's registration, pedigree and other applicable documents (see chart below) together with your payment and your signed entry form on or before the deadline date to:

b) All payments must be in US Dollars. Make checks payable to: Cedar Hill Working Dog Club (No Refunds)

d) Entries not received or postmarked by the event deadline date (Saturday, May 1, 2010) will not be accepted.

Required Documents:

A copy of ALL PAPERWORK must be submitted with the entry - (as indicated by - X) 

X* - AKC, SV, USA, or other FCI recognized registration will be accepted.
X** - Only German Shepherd's shown for the Breed Survey must be USA Registered.
          -  Registration may be completed at check-in.
X*** - USA Membership is only required for the Breed Survey.
          - Membership application may be completed at check-in.
